The fundamental principle involves an incision to remove excess tissue, followed by a re-draping of the skin and transposition of the nipple-areolar complex.
- Inferior Pedicle Technique (Wise Pattern / Anchor Incision): This is the most traditional and widely used method, particularly for very large breasts. The incision resembles an anchor, encircling the areola, extending vertically down to the inframammary fold, and then running horizontally along the fold. The inferior pedicle preserves the blood supply and nerve sensation to the nipple by keeping the tissue attached at the bottom of the breast. This technique offers excellent control over shape and projection.
- Vertical Scar Technique (Lollipop Incision): This approach involves an incision around the areola and a vertical incision down to the breast fold, eliminating the horizontal scar. It is best suited for moderate breast reductions with good skin elasticity. This method often results in a shorter recovery time and a more aesthetically pleasing scar pattern, though it may require a revision procedure to address minor puckering at the bottom of the vertical scar.
- Periareolar (Donut) Incision: Used for smaller reductions or patients with minimal ptosis, this technique involves a circular incision made only around the areola. Liposuction may be used in conjunction with this method to remove excess fatty tissue. While scarring is minimal, this technique offers less control over breast shape and is not suitable for significant volume removal.
- Free Nipple Graft: Reserved for cases of gigantomastia (extremely large breasts) or where the distance the nipple must be moved is too great to maintain a blood supply via a pedicle. In this technique, the nipple-areolar complex is completely removed and grafted onto the new position higher on the breast mound. This results in permanent loss of nipple sensation and the inability to breastfeed.
Recovery Timeline
Recovery from breast reduction is a gradual process. While patients often return to sedentary work within one to two weeks, full recovery and the final appearance of the breasts can take several months to a year.
- Immediate Post-Op (Days 1–3): Patients will experience soreness, swelling, and bruising. Dressings and bandages will be applied to the incisions, and a surgical bra or elastic bandage may be used to minimize swelling and support the breasts. Small drains may be placed to remove excess fluid or blood.
- First Two Weeks: Pain is managed with oral medication. The surgical bra must be worn continuously (except when showering). Stitches are usually dissolvable, but external sutures or skin glue may be removed after 7 to 10 days. Patients should avoid lifting heavy objects or raising their arms above the head.
