A brow lift, clinically referred to as a forehead lift or browplasty, is a cosmetic surgical procedure designed to elevate the eyebrows to a more aesthetically pleasing position and reduce the appearance of aging in the upper third of the face. As individuals age, the effects of gravity, loss of skin elasticity, and repeated muscle contraction cause the forehead and eyebrows to descend. This descent often results in a perpetually tired, sad, or angry expression, even when the patient is well-rested. The procedure aims to restore a more youthful, refreshed contour to the forehead and upper eyelids.
The anatomy of the forehead is complex, involving the skin, subcutaneous tissue, the galea aponeurotica, the frontalis muscle, and the retro-orbicularis oculi fat (ROOF). The surgical approach varies depending on the patient's anatomy, but the fundamental goal remains consistent: to reposition the soft tissues of the forehead and modify the underlying muscles that cause dynamic wrinkles. By addressing the brow position, surgeons can also ameliorate lateral hooding of the upper eyelids—the excess skin that bunches up on the outside of the eyes—without necessarily requiring a separate blepharoplasty.
Modern brow lift techniques have evolved significantly. While the traditional "coronal" lift involved a long incision across the top of the head, contemporary approaches favor endoscopic and limited-incision methods. These advanced techniques minimize scarring, reduce sensory nerve damage, and shorten recovery times. The procedure is frequently performed in conjunction with facelifts and eyelid surgeries to achieve a harmonious facial rejuvenation.
Identifying the ideal candidate for a brow lift requires a thorough evaluation of physical health, anatomical structure, and aesthetic goals. While the procedure is generally safe for a wide demographic, specific characteristics predict better outcomes and higher patient satisfaction.
While age is not a strict barrier, most candidates are between 40 and 65 years old. However, younger individuals with hereditary low brows may also benefit. Ideal candidates should be non-smokers or willing to quit several weeks before and after surgery, as nicotine constricts blood vessels and significantly impairs healing. Patients with medical conditions such as uncontrolled hypertension, bleeding disorders, or history of poor wound healing may require medical clearance. Furthermore, individuals with very high hairlines or thinning hair may require specific surgical adjustments to avoid visible scarring or further recession of the hairline.
The choice of surgical technique is dictated by the patient's hairline height, skin elasticity, and the degree of correction required. Surgeons may utilize one of four primary methods, each offering distinct advantages regarding incision placement and tissue manipulation.
The coronal lift is the traditional approach, involving an incision made from ear to ear, hidden within the hairline, slightly behind the hair-bearing scalp. The surgeon lifts the forehead skin, removes excess tissue, and adjusts the muscles beneath. This technique offers excellent exposure and is particularly effective for patients with extremely high foreheads or significant skin laxity. However, it results in a longer recovery period and a higher risk of scalp numbness (paresthesia) due to the extensive elevation of the skin flap.
Similar to the coronal lift, the pretrichial incision runs across the top of the head but is placed directly at the hairline rather than within it. This approach is ideal for patients with a naturally high hairline, as the scar is hidden at the border of the hair. By removing skin at the hairline, the surgeon can effectively lower the hairline, reducing the vertical height of the forehead while lifting the brow. The trade-off is the potential for a visible scar if the incision does not heal optimally.
The endoscopic technique is the current standard for many cosmetic surgeons. It involves making three to five short incisions (less than an inch long) behind the hairline. An endoscope—a tiny camera with a light—is inserted to allow the surgeon to visualize the underlying structures on a monitor. Specialized instruments are used to release the tissues, weaken the muscles that cause frown lines, and suspend the brow in a higher position using sutures or temporary fixation devices (such as Endotine). This method minimizes scarring, reduces bleeding, and shortens recovery time, though it may not be suitable for patients requiring large amounts of skin removal.
This "mini-lift" focuses solely on the outer (lateral) third of the brow, which is often the first area to droop. Incisions are made in the temporal hairline. It is less invasive than a full brow lift and is often performed in conjunction with an upper blepharoplasty. It is best suited for patients with mild to moderate brow ptosis who do not require correction of the central forehead or glabella.
Recovery from a brow lift is generally straightforward, though the timeline varies based on the surgical technique used. Patients undergoing an endoscopic lift typically recover faster than those undergoing an open coronal lift.
Immediately after surgery, the head is usually wrapped in a bulky dressing to minimize swelling. Patients may experience mild to moderate discomfort, which is managed with oral pain medication. Swelling and bruising are common and often extend down to the eyelids and cheeks due to gravity. Cold compresses are recommended to keep swelling at bay. It is crucial to keep the head elevated at all times, even while sleeping. Numbness in the forehead and scalp is expected and temporary. Sutures or staples are typically removed within 5 to 10 days.
Most patients feel comfortable returning to work and social activities after 10 to 14 days, assuming the swelling has subsided enough to be concealed with makeup or sunglasses. The sensation of itching (pruritus) on the scalp or forehead is common as nerves begin to regenerate. Patients should avoid strenuous exercise, heavy lifting, and bending over for at least three weeks to prevent bleeding or increased swelling. The brows may initially appear slightly higher than the final intended position; this is intentional to account for the natural settling of tissues.
By the third month, the majority of the swelling has resolved, and the true results of the lift become visible. Incision lines continue to fade and flatten over the course of a year. Nerve sensation usually returns gradually, though some areas of permanent numbness may persist near the incision sites. The results of a brow lift are long-lasting, often enduring for 5 to 10 years or more, although the natural aging process will continue.
As with any surgical procedure, a brow lift carries inherent risks. While serious complications are rare when performed by a qualified board-certified plastic surgeon, patients must be informed of potential adverse outcomes.
The cost of a brow lift varies widely depending on the geographic location, the surgeon's expertise, the complexity of the procedure, and the facility fees. On average, the cost of a brow lift in the United States ranges from $3,500 to $10,000.
Because a brow lift is primarily considered a cosmetic procedure, health insurance typically does not cover the costs. However, if the procedure is deemed medically necessary to correct visual field obstruction caused by severe brow ptosis, insurance may provide coverage. In such cases, pre-authorization and visual field testing are required to prove the medical necessity.
