A chemical peel is a dermatological procedure used to improve the appearance of the skin on the face, neck, or hands. It involves the application of a chemical solution to the skin, which causes controlled trauma or injury to the epidermis and, depending on the depth, the dermis. This deliberate damage causes the outer layers of skin to exfoliate and eventually peel off. Once the treated skin peels away, the new, regenerated skin is typically smoother, less wrinkled, and more even in tone and texture.
From a clinical standpoint, chemical peels are categorized by the depth of penetration into the skin layers: superficial, medium, and deep. The mechanism of action varies based on the agents used. Generally, the chemicals disrupt the bonds between old skin cells (desmosomes), allowing them to shed. In deeper peels, the chemicals induce protein coagulation in the papillary or reticular dermis, stimulating collagen production and remodeling. This procedure is effective for treating a variety of dermatological conditions, including acne vulgaris, melasma, photoaging (sun damage), rhytides (wrinkles), and superficial scarring.
Determining candidacy for a chemical peel requires a thorough evaluation of the patient’s skin type, medical history, and aesthetic goals. Ideal candidates generally have realistic expectations regarding the outcome and are in good physical health without active skin infections in the treatment area.
The technique employed depends entirely on the desired depth of the peel. Prior to the procedure, the skin is thoroughly cleansed with an agent such as acetone or alcohol to remove oils and debris. The chemical agent is then applied systematically, often using gauze, cotton swabs, or brushes.
Also known as "lunchtime peels," these utilize mild acids such as alpha-hydroxy acids (glycolic, lactic, or fruit acids) or beta-hydroxy acids (salicylic acid). The solution penetrates only the epidermis. The application technique involves applying the acid for a specific duration (often 3 to 7 minutes) until a slight erythema (redness) appears. The acid is then neutralized or washed off. These peels improve the texture of rough, dry skin and refresh the complexion.
Medium-depth peels typically utilize Trichloroacetic acid (TCA) in concentrations ranging from 15% to 35%, often combined with glycolic acid. The technique involves applying the solution to the face, watching for the "frosting" effect—a whitening of the skin that indicates protein coagulation and penetration into the upper dermis. Unlike superficial peels, medium peels often do not require neutralization, as the chemical is self-limiting. This depth is effective for treating dyschromia, actinic keratosis, and superficial wrinkles.
Deep peels are the most aggressive and involve the use of high-concentration TCA (above 50%) or Phenol. Due to the intensity of the pain and the potential for systemic toxicity (particularly cardiac arrhythmias associated with phenol), this procedure is often performed in a surgical setting with monitoring and IV sedation. The technique is slow and methodical, applying the chemical in segments to ensure even penetration. The frosting is immediate and opaque. Deep peels reach the reticular dermis and are used to treat deep wrinkles, severe sun damage, and precancerous growths. Because phenol can bleach the skin, this technique is generally not recommended for patients with darker complexions.
The recovery period varies significantly based on the depth of the peel. Patients must follow a strict regimen of cleansing, moisturizing, and applying ointments to aid the healing process.
While chemical peels are generally safe when performed by a qualified professional, they carry inherent risks ranging from minor temporary side effects to permanent complications.
The financial cost of a chemical peel is highly variable and depends on the depth of the peel, the geographic location of the practice, and the expertise of the provider (dermatologist vs. aesthetician). Because chemical peels are considered cosmetic procedures, they are typically not covered by medical insurance unless they are being performed to treat precancerous skin conditions or remove lesions.
