FMS is rarely a single operation; it is usually a combination of surgeries performed either in a single session or staged over time. The specific techniques employed depend on the patient's anatomy and desired masculine features.
A masculine forehead typically features a brow ridge (supraorbital ridge) and a slightly sloping vertical forehead. In contrast, feminine foreheads are often flat and vertical. To achieve this, surgeons may perform forehead augmentation using custom-made porous polyethylene implants or hydroxyapatite cement. The material is placed over the orbital rims to create projection. In some cases, the bone may be shaved down if the forehead is too protruding in the upper area, followed by augmentation of the brow ridge. This procedure is often performed via a coronal incision across the top of the head or an endoscopic approach.
Male noses are typically larger, wider, and have a straighter bridge compared to female noses.
