There are two primary methods of harvesting follicular units for transplantation: Follicular Unit Transplantation (FUT) and Follicular Unit Extraction (FUE). Both methods involve the transplantation of naturally occurring groups of 1 to 4 hairs, known as follicular units.
Also known as the "strip" method, FUT involves surgically removing a strip of scalp tissue from the donor area, usually from the back of the head. The strip is typically 6 to 10 inches long and 1 to 2 millimeters wide. Once removed, the incision is closed using sutures or staples, leaving a linear scar. The surgical team then dissects the strip under high-powered stereomicroscopes to isolate individual follicular units.
Advantages: FUT allows the surgeon to harvest a large number of grafts in a single session, making it the preferred choice for patients requiring extensive coverage. It also generally results in a higher yield (percentage of surviving grafts) because the dissection is done under microscopic control, minimizing transection (cutting the follicle root).
FUE is a minimally invasive harvesting method where individual follicular units are extracted directly from the donor area one by one. The surgeon uses a micro-punch tool, typically ranging from 0.7mm to 1.0mm in diameter, to make a small circular incision around the follicle. The follicle is then gently pulled out with forceps.
Advantages: FUE does not leave a linear scar. Instead, it leaves tiny, dot-like scars that are barely visible, allowing the patient to wear their hair very short. There is also no scalpel incision or stitches required in the donor area, resulting in less post-operative discomfort and a faster return to physical activity.
Disadvantages: FUE is more time-consuming than FUT and may require multiple sessions to harvest the same number of grafts. There is also a higher risk of transection during the extraction process if the angle of the hair is not judged correctly.
