The body’s immune system, specifically the lymphatic system, then recognizes these smaller particles as foreign debris and gradually flushes them out of the body over the weeks following treatment.
Historically, continuous-wave lasers were used, but they often caused excessive thermal damage to the skin, leading to scarring. Modern medicine utilizes Q-switched lasers (nanosecond pulses) and, more recently, Pico lasers (picosecond pulses). Pico lasers deliver energy in trillionths of a second, creating a photo-acoustic effect rather than just a thermal effect. This allows for even more efficient shattering of ink particles with fewer side effects and is particularly effective on stubborn blue and green inks.
While laser technology has advanced significantly, not every individual or every tattoo is a perfect candidate for complete removal. Several factors determine the efficacy of the procedure:
Prior to the procedure, a thorough medical history and physical examination of the tattoo are conducted. The area is cleaned, and photographs are taken for documentation. To manage discomfort, a topical anesthetic cream (such as lidocaine, prilocaine, or tetracaine) is applied approximately 30 to 60 minutes before the session. In some cases, local anesthesia injections or cooling devices (such as a cryogen spray or cold air) may be used during the procedure to numb the skin and protect the epidermis.
The patient and the medical staff wear protective eyewear to shield their eyes from the intense laser light. The handheld laser device is placed against the skin, and the practitioner activates the laser, delivering pulses of light. Patients often describe the sensation as similar to a rubber band snapping against the skin, or the splatter of hot grease.
The duration of the session depends on the size of the tattoo. Small tattoos may take only a few minutes, while larger sleeves or back pieces can take significantly longer. During the process, the tattoo may turn white or frost over—a phenomenon known as frosting. This is caused by the formation of gas bubbles (carbon dioxide) under the skin and is a sign that the laser energy is effectively breaking up the ink.
Recovery from laser tattoo removal is relatively quick compared to surgical excision, but it requires strict adherence to aftercare protocols to prevent infection and scarring.
Immediate Aftercare (Days 1–3): Following the treatment, the area will likely be red, swollen, and tender. The "frosting" usually subsides within 24 hours. The site may feel sunburned. It is crucial to keep the area clean and dry. An antibiotic ointment or a healing balm (such as petroleum jelly) is typically applied to keep the wound moist. Ice packs can be used to reduce swelling.
Healing Phase (Days 4–14): As the skin heals, blisters or scabs may form. It is imperative not to pick, scratch, or pop these blisters, as doing so can lead to scarring and infection. The treated area should be covered with a sterile bandage when exposed to dirt or clothing. The scabs will naturally slough off, revealing pink, fresh skin underneath.
Long-Term Interval (Weeks 6–8): The body continues to flush out ink fragments during this time. Fading becomes noticeable as the inflammation resolves. Patients must avoid sun exposure strictly, as UV rays can cause hyperpigmentation (darkening) of the treated area. Once the skin has fully healed, the next session can be scheduled. Most tattoos require 6 to 12 sessions, spaced 6 to 8 weeks apart.
While laser tattoo removal is generally safe when performed by a qualified professional, there are inherent risks associated with the procedure.
The cost of laser tattoo removal varies widely based on geographic location, the expertise of the provider, and the complexity of the tattoo. Unlike getting a tattoo, removal is charged by the session or by the square inch, rather than a flat fee for the design.
