While PRP is a versatile treatment, it is not a universal cure for all types of hair loss. Identifying the ideal candidate is crucial for managing expectations and ensuring clinical success. The therapy is most effective for individuals currently in the early stages of hair thinning or those experiencing active shedding.
Conversely, candidates with diffuse, scarring alopecias (cicatricial alopecia) or those with completely bald slick areas may not be suitable candidates. Additionally, individuals on blood thinners, anticoagulants, or those with chronic medical conditions such as liver disease, platelet dysfunction, or active infections may be disqualified from undergoing the procedure.
PRP hair restoration is a minimally invasive, in-office procedure that typically takes between 60 to 90 minutes. The protocol follows a strict three-step process to ensure the potency of the plasma is maintained.
1. Phlebotomy and Blood Collection: The process begins with the venipuncture of the patient's arm, typically drawing between 20 to 60 milliliters of blood. The volume depends on the size of the treatment area and the specific concentration system being utilized.
2. Centrifugation and Processing: The blood sample is placed in a sterile centrifuge. This machine spins the blood at high speeds to separate its components based on density. The goal is to isolate the buffy coat, a thin layer of white blood cells and platelets suspended between the red blood cells at the bottom and the platelet-poor plasma (PPP) at the top. Advanced closed-system kits are often used to prevent contamination and ensure a sterile environment. The resulting plasma typically contains a platelet concentration 4 to 6 times higher than baseline physiological levels.
3. Activation and Injection: The concentrated PRP is prepared for injection. Some protocols involve adding calcium chloride or thrombin to activate the platelets immediately, causing the gel formation and release of growth factors. Others rely on the collagen in the scalp to activate the platelets naturally upon injection. Using a fine-gauge needle (typically 30-gauge), the physician injects the PRP directly into the mesoderm layer of the scalp at the level of the hair bulbs. Injections are administered in a grid pattern, approximately 0.5 to 1 centimeter apart, covering the entire thinning area. To enhance efficacy, microneedling may be performed prior to injection to create micro-channels that facilitate better absorption of the growth factors.
One of the primary advantages of PRP therapy is the minimal downtime associated with the procedure. However, the physiological changes in the scalp occur over several months.
To maintain results, a series of treatments is generally required. Most protocols suggest an initial loading phase of three treatments spaced 4 to 6 weeks apart, followed by maintenance sessions every 4 to 6 months.
Because PRP is an autologous treatment (using the patient's own blood), the risk of allergic reaction, rejection, or disease transmission is virtually non-existent. However, as with any injection-based procedure, there are inherent risks.
The cost of PRP hair restoration varies widely based on geographic location, the expertise of the provider, and the specific PRP system used. Generally, PRP is considered an elective, cosmetic procedure and is not covered by medical insurance.
Per session costs typically range from $500 to $2,500. Patients should be wary of providers offering significantly lower prices, as this may indicate the use of lower-quality centrifugation equipment or a lack of proper sterile protocols.
