Sclerotherapy is widely regarded as the gold standard for the treatment of spider veins (telangiectasia) and small varicose veins. It is a minimally invasive medical procedure used to eliminate varicose veins and spider veins. Sclerotherapy involves the injection of a solution (commonly called a sclerosant) directly into the affected vein. The solution irritates the lining of the blood vessel, causing it to swell and stick together. Over time, the vessel turns into scar tissue that fades from view.
From a pathophysiological perspective, spider veins occur when the valves within the veins weaken or fail. In healthy veins, these valves ensure blood flows toward the heart. When they malfunction, blood can flow backward (reflux) and pool in the vein, causing pressure and the bulging of delicate vessels visible on the skin's surface. While often considered a cosmetic concern, they can cause symptoms such as aching, burning, swelling, and night cramps.
The mechanism of action for sclerotherapy is chemical ablation. The injected sclerosant—commonly sodium tetradecyl sulfate (STS), polidocanol, or hypertonic saline—induces endothelial damage. This damage triggers an inflammatory response, leading to fibrosis and the eventual collapse of the vein lumen. Once the vein is sealed, blood is rerouted through healthier, deeper veins, and the collapsed superficial vein is absorbed by the body’s natural metabolic processes.
While sclerotherapy is a versatile procedure, it is not suitable for everyone. Identifying the ideal candidate is crucial for achieving optimal results and minimizing adverse events.
Contraindications for sclerotherapy include pregnancy, breastfeeding (due to lack of safety data for sclerosants), a history of deep vein thrombosis (DVT), or patients who are bedridden. Additionally, individuals with allergies to the sclerosing agent or those with severe arterial disease should not undergo the procedure.
Although sclerotherapy is not "surgery" in the traditional sense of scalpels and incisions, it is a precise medical technique requiring skill and anatomical knowledge. The procedure is typically performed in an outpatient setting and takes less than 30 minutes.
1. Pre-Procedure Assessment: Before treatment, the physician will conduct a physical examination and potentially a duplex ultrasound. This imaging test maps the blood flow and rules out underlying venous insufficiency in the saphenous vein, which must be treated before addressing surface spider veins to prevent recurrence.
2. Preparation: The patient stands so the doctor can mark the specific veins to be treated. The patient then lies down (supine position), and the skin is cleaned with an antiseptic solution such as alcohol or chlorhexidine.
3. The Injection: Using a fine-gauge hypodermic needle (typically 30-33 gauge), the physician injects the sclerosant directly into the vein. The volume of injection depends on the size and length of the vein.
4. Compression and Post-Injection: Immediately after the needle is withdrawn, cotton balls and compression tape may be applied to the injection sites. In many cases, the patient is fitted with graduated compression stockings. Compression is vital as it helps keep the vein walls apposed (pressed together), preventing blood from flowing back into the treated vessel while it scleroses.
Recovery from sclerotherapy is generally rapid, allowing patients to return to their normal daily activities immediately. However, the complete aesthetic resolution takes time.
Patients should avoid heavy exercise, hot baths, saunas, and direct sun exposure (which can cause hyperpigmentation) for at least two weeks post-procedure.
Sclerotherapy is a safe procedure with a low risk of serious complications when performed by a qualified practitioner. However, side effects can occur.
