Top surgery is a gender-affirming surgical procedure designed to modify a person's chest to align their physical appearance with their gender identity. While the term is often used broadly, it specifically refers to two distinct categories of procedures: Female-to-Male (FTM) top surgery (chest masculinization or mastectomy) and Male-to-Female (MTF) top surgery (breast augmentation or mammoplasty). For transgender men, transmasculine individuals, and non-binary people, the goal is the removal of breast tissue and contouring of the chest to appear masculine or flat. For transgender women, transfeminine individuals, and non-binary people, the goal involves increasing breast size and shape to achieve a feminine aesthetic.
From a clinical standpoint, these procedures are major surgeries that require general anesthesia. They are considered highly effective treatments for gender dysphoria, significantly improving mental health outcomes and quality of life for patients. The specific surgical approach is determined by factors such as skin elasticity, chest wall anatomy, the volume of existing breast tissue, and the patient's desired aesthetic outcome regarding nipple placement and scarring.
Ideal Candidates
Ideal candidates for top surgery are individuals who have been diagnosed with gender dysphoria and seek physical alteration of the chest to alleviate psychological distress. However, clinical suitability extends beyond gender identity to include physical health and lifestyle factors.
- Age and Maturity: Surgeons typically require patients to be at least 18 years of age, though some exceptions exist for minors with parental consent and stringent psychological evaluation. Candidates must possess the emotional maturity to understand the permanence of the procedure and the limitations of surgical outcomes.
- Health Status: Candidates should be in good general health. Chronic conditions such as uncontrolled diabetes, heart disease, or bleeding disorders can increase surgical risks significantly. A body mass index (BMI) within a healthy range is often preferred, as obesity can complicate anesthesia and increase the risk of wound healing issues.
- Smoking and Substance Use: Non-smokers are ideal candidates. Nicotine constricts blood vessels, which drastically increases the risk of necrosis (tissue death), particularly in nipple grafts. Most surgeons require patients to cease smoking for several weeks before and after surgery.

MTF Breast Augmentation Techniques
MTF top surgery involves increasing breast volume using implants or fat transfer.
- Implant-Based Augmentation: This is the standard approach. The surgeon creates a pocket either behind the breast tissue (subglandular) or behind the pectoralis muscle (submuscular). Submuscular placement generally offers a more natural look and reduces the risk of visible implant edges, particularly in patients with little existing breast tissue. It avoids the use of foreign objects but offers less predictable volume retention.
Recovery Timeline
Recovery is a progressive process that varies by individual and the extent of the surgery performed.
- Immediate Post-Op (Days 1–3): Patients wake up with surgical drains (common in FTM surgery) and a compression binder or surgical bra wrapped around the chest. Pain is managed with prescription medication. Rest is strictly enforced, and arm movement must be minimized to prevent tension on the incisions.
- First Two Weeks: Drains are usually removed within 3 to 7 days. Patients must wear the compression garment 24/7 (except when showering) to minimize swelling and fluid accumulation. Soreness is significant, and raising the arms above the head is restricted. Most patients can return to sedentary work or school after 1 to 2 weeks.
- Weeks 3–6: Strenuous activity and heavy lifting (over 5-10 lbs) are prohibited. Light walking is encouraged to prevent blood clots. Swelling (edema) begins to subside, and the chest starts to settle into its new shape. Numbness in the chest area is normal; sensation may gradually return over months.
- Months 3–6: Patients can typically resume upper-body workouts and cardio. Scars begin to mature, initially becoming red and raised before eventually flattening and fading. For FTM patients with free nipple grafts, full sensation rarely returns, but protective sensation (feeling pressure) usually does.
- Year 1 and Beyond: The final result of the surgery is usually apparent at the one-year mark. Scars will continue to fade for up to 18 months. Routine mammograms or cancer screenings should still be conducted as per medical guidelines for the remaining tissue.
Risks and Complications
